Supports for older adults * helps older adults remain independent in their own homes for as long as possible

Caregiver support and other homemaking services:

Adult day programs
care coordination
client intervention and assistance
Meals on Wheels * frozen meals delivered once a week
homemaking
assisted living services * 24 hour in home support for high risk seniors
