Description of Services:
Education, Self-Management and Exercise Program for adults with history of cardiac disease
- Offers a 3-month program for local patients; 6 month program for Northern patients
- Provides assessment, stress test, exercise prescription, exercise sessions, and discharge completed on site
- Option for in-home program (Northern patients)
- Telemedicine connection for education sessions
- Access to dietitian, social worker, and smoking cessation
