Description of Services:
Health care services offered at home, at school, and in the community, as well as access to supported living programs and long-term care * care coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care * provides information about other community services and refers when appropriate

Specialized services may include:
Information and Referral - CentralWesthealthline.ca provides online access http://www.centralwesthealthline.ca/ to health, social and community resource information for Brampton, Caledon, Dufferin County, Etobicoke North, Malton and West Woodbridge area
School Health Support Services - Special health services for children who require medical or rehabilitative assistance to attend school, participate in school routines and receive instruction * services are provided within public, private or home-based schools
Mental Health and Addictions School Program - Assessments, interventions and support to students and families with mental health and or addiction issues * consultation to school staff in regards to mental health and/or addiction issues as appropriate
Placement Services - Determine eligibility for admission to long-term care facilities and prioritize and manage the admission process * support and information is provided to applicants and their families prior to and during the admission process
Community Nursing Clinics - Nursing services for ambulatory patients at clinics located in the community
Supported Living - Information and/or assessment for Adult Day Programs (ADS), supports for Daily Living or Supportive Housing
Health Care Connect - Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector * Care Connectors work with family doctors and nurse practitioners who are accepting new patients
Services available to eligible patients may include:
- nursing
- nurse practitioner
- personal support
- physiotherapy and occupational therapy
- speech and language therapy
- social work
- nutrition and food counselling (dietitians)
- medical supplies and equipment
- referral to other community supports
Specialized services may include:
- access to supported living (adult day programs, supportive housing and assisted living)
- access to respite care and convalescent care (short-term stays)
- access to long-term care homes
- health support in the school
- specialized services for people living with specific health conditions
- palliative care
