Description of Services:

A specialty geriatric mental health outreach team * PASE offers specialized geriatric mental health outreach services for the four counties of Northumberland, City of Kawartha Lakes, Haliburton and Peterborough * The program has Ontario Televideo capacity and offers follow-up services and urgent response service to Haliburton LTC homes using a specific protocol * Virtual care and telephone assessments offered

Services include:
  • Psychogeriatric Resource Consultants (PRC)
  • Intake and screening
  • Specialized assessment and consultation to referral sources of clients (who have a family physician)
  • Care Planning, short-term treatment and follow-up
  • Recommendations re treatment and service needs
  • Consultation/education and support to caregivers and service providers
  • Program and system development/Liaison with other professionals, groups and planning bodies to integrate and enhance services for seniors
  • Education and knowledge transfer for staff in long-term care facilities and community agencies that serves seniors experiencing mental health problems
  • Consultation and support for staff in long-term care facilities and community agencies in the application and interpretation of assessment strategies and in the initiation of care plans and interventions
  • Assistance in the development and maintenance of local area dementia networks and to serve as a resource for existing networks. Note: The Psychogeriatric Resource Consultant role does not include direct clinical contact serve registered clients; carry out assessments, developing care plans or other care-related activities.
  • Exclusion criteria - PASE does not do assessments for legal purposes including capacity
  • Through negotiated agreement with Ontario Shores, (then WMHC) Ontario Shores is responsible to provide clinical serves to LTC homes in the county of City of Kawartha Lakes and PASE serves clients living in the community of City of Kawartha Lakes.
