Referrals can be made directly by individuals, physicians, community service providers or concerned family members.
Telephone or fax referrals are accepted.

Inclusion criteria:
Individuals 60 years of age or older who:
- Are experiencing a mental health issue (or symptoms from a previously diagnosed mental health issue)
- Display significant changes in mood, behaviour or cognition
- Have difficulty managing safely due to changes in their mental function
- Are medically stable
- Reside in the community (including a retirement residence), excluding long-term care

Description of Services:
Mental health service for older adults (60 years and older) with concerns about memory loss, mood, anxiety, stress, loss, or a previously diagnosed mental health disorder.
This service cannot provide urgent medical care or crisis services.
Service includes:
- in-home mental health assessment(s) with individualized recommendations for clients who meet triage criteria
- support in-person, by phone or online regarding resource navigation and caregiver consultation
- cognitive testing
- indirect psychiatric consultation model (assessment by mental health clinician, which is reviewed with geriatric psychiatrist consultant); note: client is not seen directly by psychiatrist
- development of a care plan
- recommendations and information about management and coping with mental health and/or cognitive changes
- referrals or linkages to other community services or mental health programs
This service is also helpful for care partners experiencing stress who would like information about local home and community services or guidance on the process of applying for long-term care.
