Description of Services:
An inter-disciplinary team of health care providers who provide coordinated medical, nursing and psycho-spiritual care services as well as bereavement care services to individuals facing life-threatening illness and their family
Team works in consultation with the family physicians, Home and Community Care, community service provider agencies, West Lincoln Memorial hospital, Long term-care facilities and McNally House Hospice * team travels to meet people in the most appropriate setting of care
