To qualify for the government-funded Designated Physiotherapy Clinic (DPC) program, patients must be:

• Individuals requiring physiotherapy services that have been referred by a physician or nurse practitioner for an acute (new) change in their physical status
  
  
• Must be insured under the Health Insurance Act and have a valid Ontario Health Card
  
  
• Must be within one of the following categories:
  
  
  • Aged 65 or older
    
    
  • Aged 19 or younger
    
    
  • recently discharged as in inpatient of a hospital for conditions directly connected to the condition, illness or injury for which they were admitted
    
    
  • Eligible for Ontario Disability Support Program (ODSP) or the Ontario Works (OW) program

Note: Patients currently receiving physiotherapy funding under Workers Compensation, Motor Vehicle Accident insurance or other third party payers, and patients with chronic conditions that are unchanging are not considered eligible for this program.
