Description of Services:

Apartments with 24-hour personal care
  • light housekeeping
  • laundry support
  • meal preparation and support
  • reassurance and security checks
  • medication reminders
  • access to transportation and support for weekly grocery shopping trip
  • emergency response button
  • social and recreational programs   
  • must live on location – TCH Building – 7/11 Arleta Ave

  • Case Management Services Assist client in achieving and maintaining recovery through the development of comprehensive care plans * linking with appropriate community resources * supporting clients with system navigation, crisis intervention, life skills, service coordination and psychosocial support
  •  
  • Reintegration Program Transitional housing support to older adults with serious mental health and addiction challenges who are presently in hospital (ED/ALC patients) and require transitional support services prior to achieving permanent housing in the community
