Description of Services:

Transitional housing support to older adults with serious mental health and addiction challenges who are presently in hospital (ED/ALC patients) and require transitional support services prior to achieving permanent housing in the community

Referrals received through HRRH * Psychogeriatric Case Managers (PCM) assist clients in achieving and maintaining recovery through the development of comprehensive care plans * linking with appropriate community resources * supporting clients’ transition into permanent housing in the community * On-site 24 hour Supportive Housing Services are provided to reintegration clients Provides transitional housing support to older adults with serious mental health and addiction challenges who are presently in hospital (ED/ALC patients) and require transitional support services prior to achieving permanent housing in the community * referrals received through HRRH * Psychogeriatric Case Managers (PCM) assist clients in achieving and maintaining recovery through the development of comprehensive care plans * linking with appropriate community resources * supporting clients’ transition into permanent housing in the community * On-site 24 hour Supportive Housing Services are provided to reintegration clients
