Description of Services:
Ontario Health atHome-Brampton (Central West) Nursing Clinic.
Integrated Care Clinics provide a variety of specialized nursing services to patients of all ages. Depending upon the specific clinic, these services may include:
Wound care
Drain care
Injections
Central line maintenance
Catheter care
Enterostomal therapy/ostomy care and education (ET nurse available to see patients at the clinic once per week)
IV therapy and IV access maintenance
Chemotherapy connect and disconnects
Lymphedema care
Diabetes management counselling and education
Education on self-management including but not limited to diabetes and injection
