Description of Services:

Program that refers people without a regular family health care provider to physicians and nurse practitioners who are accepting new patients in their area

voluntary program for both patients and participating family health care providers - patients register by calling 811, with a centrally located patient representative, or they can register online www.ontario.ca/healthcareconnect
patients must have a valid Ontario Health Card Number to be eligible for the program
once patients are registered, information is forwarded to Ontario Health atHome office that covers the area where the patient resides
Care Connectors are based locally in Ontario Health atHome-Brampton offices and facilitate the connection between patients and doctors/nurse practitioners
